Name change after bankruptcy?

I have a general question about changing my name. I was married outside of the U.S but did not change my name when I returned. A year later I filed for bankruptcy. I am aware that I cannnot change my name for fraudulent reasons but I would like to add to my name for religious purposes. I would like to add an Islamic first name to my original aname and add my husbands last name as well as keep my own first and last name. Is this possible and if so where can i find the forms online?
