I received a bill information Notice from a Hospital, "nnnnnn Health", dated 07/20/96 Re: Inpatient Psychiatry Care for a Joe L. Blo, I am Joe X. Blo. His birthday is 03/28/62 making him 17 yrs younger than I am. Both day and month of the dates are totally different. His address is in another town and county , 119 miles away, according to the records on the Notice.
Why would they even bill me and my Insurance Provider in the first place when the details are so different.
Assuming a 'Good Faith' error, I called the Customer Service number provided, on 07/26/96 and informed them that they had the wrong person and requested a correction.
Fast forward to 11/15/2010, I am changing my primary care provider, I am sitting in his office for my initial visit with him, going over my Medical Record and on some Database that he has access to, up pops a field in my records for Psychiatric Services. No other details could be extracted.
Now in many business activities, insurance or bond is carried against E&O's, (Errors and Omissions), to cover against liabilities or losses resulting from cause mentioned. I am a Professional working in the Health & Scientific related areas, to what extent this entry may have damaged my professional mobility I cannot precisely quantify, but what compensatory remedies do I have available?

PS. I think there is a significant "Fraud" component involved here which I will pursue, because I am inclined to believe that despite my informing them I doubt whether they made any attempt to retract the bill or repay my insurance provider.
