custody of God child

I am trying to find out if there is a legal form to fill out and perhaps have notorized, granting custody of a child to the Godparents in case of parents death. The parents of the child have family but absolutely do not want any of them to obtain custody if something were to happen to them. They named my husban and myself as Godparents of their baby with the intent that we would have custody if something should happen to them.
If this were to happen I want to make sure their wishes are carried out.
I would appreciate any advice on this matter.
